A copy of your past 2 year’s tax returns (if this is your first year working with ADACI)
Proof of medical insurance (Forms 1095-A, -B, or -C)
All W-2’s, LES’, & 1099’s in all states that both of you and your spouse lived in during the tax year
All statements of charitable contributions donated to qualified nonprofits
Statements of all passive income, including all rental property, K-1’s, etc.
